How To Use Our On-line Request:
In order to utilize this on-line service you must click on the appropriate links below. Please note that after doing so you will be given a Security Warning advising you that the information you are submitting is secure. In order to proceed you must answer yes to the question; Do you want to proceed? You will than be directed to the corresponding form to complete your request.

Arranging Appointments On-line:
As a convenience to our new and established patients, routine, non emergent appointments be scheduled on-line. Simply complete our On-line Appointment Request Form  with the date and approximate time that fits your schedule and we will do our very best to accommodate you. We suggest you provide at least 2 alternative dates and times. You will receive a return email confirming your appointment. Please allow a minimum of 48 hours for our response.

Medication Refills:
For you convenience we have a on-site pharmacy or you may utilize another pharmacy of your choice. If you choose to utilize off-site pharmacies please contact us a minimum of 72 hours prior to the time you intend to pick up your medication. You can either request a refill of your medications on-line by completing a Refill Medication Request Form or you may contact us at (305) 448-8134 during normal hours of operation. In order to avoid errors we require the correct spelling of the name of the drug or drugs, dosage and quantity requested as well as the telephone number of the pharmacy. Please be certain that you contact us a minimum of 72 hours prior to the time you will be going to pick up the medication We also suggest you contact the pharmacy prior to going over to pick up the medications to be certain your order is waiting for you. Sorry, medication refills are not performed after hours or on weekends so we ask that you plan accordingly.

Arranging Referrals to Specialists:
We understand the importance of having your referrals ready for you prior to your appointments with specialists. We ask that you adhere to the following policy as it relates to referrals. All referral requests are handled by our referral coordinator. Routine referrals are obtained every Friday. We ask that you request referrals a minimum of 1 week prior to the scheduled appointment. You may contact our referral coordinator to arrange a referral by either completing our On-line Referral Request Form or by contacting her at the office (305) 448-8134 between 9:30 AM and 1:30 PM. All requests for urgent referrals should be made by telephone and are reviewed by Dr. Mones. Everything possible will be done to obtain urgent referrals as quickly as possible.

Obtaining Laboratory / Diagnostic Test Results

  • Most often it is best for you to schedule a follow up appointment within 2 to 14 days from your visit to discuss your results with the doctor or physician assistant in our office. Some tests take one day to be completed and others such as pap smears and biopsies require up to 14 days. The doctor or medical assistant can best advise you at the time the tests are obtained as to when to come back in for your results.
  • As an alternative you may click on the following link, Request Lab Results by Email, to receive your results by email. We will do our very best to email your results to you within 24 to 48 hours.
  • Call our office at 305 448-8134 and leave a voicemail message for one of our medical assistants and they will do their best to return your call within 3 to 4 days. Due to the volume of calls we receive daily, we would greatly appreciate you using this method as a last resort.
